    RELEASE OF LIABILITY AND WAIVER AGREEMENT
    (All Participants must sign; Parents/Guardians of Minors must sign for youth)
     By clicking submit you agree to the following:
    IN CONSIDERATION of my involvement and participation at the Kentucky State Target Archery Association event, I acknowledge and agree that:
     
    (1)  I risk bodily injury, including paralysis, dismemberment, disability and death, and while particular rules of my sport, equipment, personal training and discipline may reduce this risk, the risk of injury does exist, as well as the risk of damage to or loss of property;
     
    (2)  I knowingly and freely assume all such risks, both known and unknown, even if arising from negligence of the releasees or others;
     
    (3)  I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual or unnecessary hazard during my presence or participation, I will bring such to the attention of the nearest official immediately; and
     
    (4)  I, for myself and on the behalf of my heirs, assigned, personal representatives, and next of kin, hereby release, indemnify and hold harmless Kentucky State Target Archery Association; and their officers and board members, volunteers, officials, agents, sponsors and/or employees, with respect to any and all injury and loss arising from my participation, whether caused by the negligence of the releasees or otherwise, except that which is the result of gross negligence or wanton misconduct, to the extent permitted by the applicable law.
     
    (5) I grant to Kentucky State Target Archers Association, its representatives and employees the right to take photographs and/or videos of me and my property in connection with the above-identified subject. I authorize Kentucky State Target Archers Association, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Kentucky State Target Archers Association may use such photographs and/or videos of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

    Cancellations received on or before August 18th are subject to a 15% fee.
    Cancellations received after August 18th are non-refundable.
    In the case that this event must be canceled for reasons beyond our control, your entry will be refunded minus 10% to cover merchant fees.
